BWC Adopts New Permanent Partial Forms and Procedure

The BWC combined the C92 and C92A forms into a new C92 form, which is used to request a permanent partial award. The old C92 form was used to apply for the initial permanent partial award. The old C92A form was used to apply for an increase in the permanent partial award.

The new C92 form is used for all permanent partial applications.

If the C92 seeks an increase in the permanent partial award, the BWC may conduct either a file review or a medical exam.

Therefore, the BWC requires that an application for an increase in the permanent partial award where no new conditions have been allowed include a medical report to support the increase.

If a new condition has been allowed, no medical report is required with an application for an increase in the permanent partial award, and the BWC will schedule an examination.
